Submission of a Full Proposal is by invitation only.
If your proposal is selected to move forward in the process, you will receive an email inviting you to submit a Full Grant Proposal. The email will provide you with the link to log in to your account and access the forms for submitting a Full Proposal. It will also include a timeline, due date and if there is any additional information requested by the Grant Review Committee.

The following items are required for all proposals. Please complete the forms, upload and submit them along with your Full Proposal.
Update your Initial Application and budget form if there is new or revised information for the Grant Review Committee to consider. The Initial Application and budget forms can be updated by logging into your account.
A Logic Model helps identify and prioritize the aspects of your program that are most critical for tracking and reporting on your grant. The logic model is meant to help develop the best course of action and provide benchmarks for measuring success.
The logic model should incorporate the measurable goals and/or outcomes that were identified in your Initial Application.

In order to maximize the impact of your proposal, please identify any foreseeable risks and steps you can take to prevent the risk from occurring and an action plan to respond to risks that occur.
Risk Assessment Form (DOC)
A grant Proposal Narrative must be included with each application. Proposal Narratives must be concise, limited to 5 pages and have a font size of 10 point or larger.
Proposal Narrative Form (DOC)
An Organization Budget must be submitted with your online application. There is no required format for the budget. If your organization is a government agency, please submit a document noting this status in lieu of the organization budget.
In your Full Proposal email invitation, you will receive a link to log in to your account and submit your Full Proposal. If you need help with your organization's account, your login or password, please contact us.
If you have an existing saved proposal, you can edit your proposal any time before the submission deadline.
Access in-progress application
You will receive a confirmation email once your application has been submitted. If you do not receive an email confirmation, verify that your application has moved from "In Progress Applications" to "Submitted Applications" in your account. You may also contact us for confirmation.
Technical assistance will be available until 1 p.m. on the deadline dates. No assistance will be available after this time. Applications will be accepted until 11:59 p.m. on the deadline date.
