Reporting Process for Behavioral Health and Early Childhood Health Grants

  1. Log in to your organization's account to begin your report. Use the login ID and password that was used when the grant application was submitted. 
  2. Complete the online reporting form.
  3. Complete and submit the following documents along with your grant report:
