For Current Grantees
A Medica staff member serves as your liaison to learn more about your project and to provide assistance if needed.
Program grant recipients are asked to submit a mid-term report mid-way through the grant period, and a final report after the grant period ends through our online portal. We will also connect with you a couple times verbally or via email for a brief update.
Rural health grant recipients will have a brief final report due at the end of their grant period.
The terms and due dates for grant reporting are determined with the organization and outlined in the grant agreement. Please contact us if you have any questions or need additional information.
How to Report on Your Grant
The Medica Foundation requires grantees to submit reports on the overall progress and financial status of their grant. The terms and due dates for reporting are outlined in your grant agreement.
Reporting Process for Behavioral Health and Early Childhood Health Grants
- Sign in to your organization's account to begin your report. Use the login ID and password that was used when the grant application was submitted.
- Complete the online reporting form.
- Complete and submit the following documents along with your grant report:
