How to Report on Your Grant

A Medica Foundation staff member serves as your point of contact to learn more about your project and to offer help if needed.

Program grant recipients will connect with a Medica Foundation staff member mid-way through the grant period and submit a narrative and financial report through our online portal after each year of funding.

Rural health grant recipients will have a brief final report due at the end of their grant period.

The terms and due dates for grant reporting are determined with the organization and outlined in the grant agreement. Please contact us if you have questions or need more information.

Sign in to report on your grant

  1. Sign in to your organization's account to begin your report. Use the ID and password you used when you submitted the grant application. If you need help contact Heather Craig.
  2. Complete the online reporting form.
